To mail in the tax payments for Form 2290, you will need to make a check or money order payable to the 'United States Treasury' and include it with your completed Form 2290 when you send it by mail.

Suspended vehicles are those that do not exceed 5000 miles (7500 for agricultural vehicles) and are exempt from the Heavy Vehicle Use Tax. Failure to file the return will result in a penalty of 4.5% of the amount due, plus an additional 0.54 cents for each mile over the 5,000 mile limit that the vehicle was used for. When filing the return, you will be required to pay the tax amount due and the penalties. Suspended vehicles are not allowed to operate on public highways until the 2290 form has been filed along with full payment to the IRS.

Form 2290 proof of payment is a document showing proof that you have paid your Heavy Vehicle Use Tax for the given tax period. Schedule 1 is proof of payment. This document is an important part of your filing and you must keep a copy of the Schedule 1 with your trucking records. Once your tax return is accepted by the IRS, it will produce a Schedule 1 paper copy that can be emailed, faxed or snail-mailed to you as proof of payment.
